Project Nevada does alter the game's mechanics, but it's hard to imagine ever going back to vanilla after using it (it includes a grenade hotkey, btw). Backpacks are a nice addition too - Nivea's Spice of Life mod is a good mod for combining tons of different little clothing mods, backpacks being just one feature. I didnt use grenades at all til I got that modded in.

I dont see any special value in preserving the vanilla experience of, say, lacking a grenade hotkey. YUP (unofficial patch) and NVSE (necessary to most complex mods), NVAC (anti-crash), NVSR (stutter reducer) - but dont be afraid to add mods as annoyances crop up. Maybe dont start with anything more than bugfixes and stability mods - i.e. I'd say just add mods according to what you want to see in the game.
